Terms of Service

1. Acceptance of terms
By accessing and using HealMaze telehealth services (the "Services"), you agree to be bound by these Terms of Service (these "Terms"). These Terms govern your relationship with HealMaze Inc., an Alberta-registered healthcare provider operating within Alberta medical standards.
2. Healthcare services
2.1 Scope. HealMaze provides virtual family-medicine care for adults living with chronic conditions and the mental-health impact of those conditions, delivered by Alberta-licensed family physicians.
2.2 Coverage. Services are billed to the Alberta Health Care Insurance Plan (AHCIP) for eligible residents with a valid Alberta Personal Health Card.
2.3 Limitations. HealMaze is not an emergency service. If you are experiencing a medical emergency or mental-health crisis, call 911 or go to your nearest emergency department.
3. Patient responsibilities
3.1 Accurate information. You must provide complete and accurate health information. Inaccurate information may affect the quality of your care.
3.2 Alberta residency. Insured services are available to Alberta residents with a valid Alberta Personal Health Card.
3.3 Technology. You are responsible for maintaining the internet connection and device needed to attend virtual visits.
4. Privacy and health information
4.1 HIA compliance. We comply with Alberta's Health Information Act and all applicable privacy legislation.
4.2 Information collection. We collect only the health information necessary to provide appropriate care.
4.3 Information use. Your health information is used only for healthcare delivery, billing, and quality improvement. See our Privacy Policy.
5. Referrals
Our physicians may refer you to specialists or allied-health services when medically appropriate. Referrals follow standard Alberta protocols and are subject to specialist availability and coverage rules.
6. Limitation of liability
6.1 Medical care. Our physicians provide care according to Alberta medical standards. Healthcare outcomes cannot be guaranteed.
6.2 Technology. We are not liable for technology failures beyond our control that may affect a visit.
7. Service modifications
HealMaze Inc. may modify, suspend, or discontinue services with reasonable notice. Changes to Alberta Health Care coverage or regulations may affect service availability.
8. Termination
Either party may end the physician–patient relationship following Alberta medical guidelines. We will provide appropriate transition of care and medical-record transfer as required.
9. Governing law
These Terms are governed by and construed in accordance with the laws of Alberta, Canada. Any disputes will be resolved through Alberta legal processes.
